Statistical thinking is the process is the data used to create knowledge and it is shared with other to effect high quality output. The Deming Theory of Profound Knowledge consist of four parts: systems thinking, process variation, theory of knowledge and psychology. “Each system is perfectly designed to deliver the results it produces” (Roehrs, 2018). Systems of thinking is the process of understanding of the system that is in place. The mapping process is a process to get all the right parts moving in the right direction. Process variation recognizes that all processes have variation that can be either inherent in the process or due to external influences. Theory of knowledge comes with combining theory and knowledge to create knowledge. Psychology deals with way people think. (Lighter, 2011). All four parts are considered as techniques used to produce the best result for the system that is being used.

How do the four elements of his theory relate to improving performance?

Managers have to have an open mind when it comes to making sure they understand the way the system works and to collect data that can be shared with others. Using statistical thinking does not mean that you have to be the smartest person, but you have to use the know how to incorporate statistical information to make sure you come out with the highest inputs.

The scripture that comes to mind is Proverbs 2:6 (NIV), “For the Lord gives wisdom; from his mouth come knowledge and understanding.” The Lord knows everything about us and he gives us the knowledge and understanding that we need to succeed.

William Edwards Deming is considered to be one of the twentieth centuries mos influential industrialists. He found his place in history during World War II and with his idea of pushing out quality before quantity. He endeavored to improve the quality of companies products as well as their leaders by initiating four “elements” into standard practice easy enough for any business to understand. The four elements were, and still are; 1) Appreciation for a system, that is, understanding how the business itself works, 2) Knowledge about variation. Deming felt that, ” Managers who do not understand variation frequently increase variation by their actions. Critical to this is understanding the two types of variation — Common cause which is variation from the system and Special cause which variation from outside the system 3) Theory of knowledge, because knowlegde and experience are two different things, and finally, 4) Knowledge of psychology in order to better understand the relationship between people and the work place.

Deming understood what it meant to “serve one another in love.” We can see this in the way he was so very concerned in quality over quantity. It is a lesson in servant hood, and an attitude we can all afford to adopt.

Organizational Assessment

I have been offered, and accepted in my career, two positions of Director of Rehabilitation in skilled nursing facilities (SNF). Prior to accepting either of these positions, I wanted to be sure that these organizations were suitable and demonstrated a good structure. Some of the issues to be looked at in healthcare organizations include those of “social context and behaviors described in culture-such as respect, appreciation, participation, and communication” (Thomas, 2017, p. 170). The structure of an organization also allows insight to how “new issues, regulatory requirements or lines of business” are handled within the organization (Soderstrom & Weber, 2020). Even as a leader of an organization it is important to remember and reflect on scripture as Philippians 2:3 says, “Do nothing from rivalry or conceit, but in humility count others more significant that yourselves” (ESV). A leader is only as strong as the organization and the team they are leading.

Shared Governance

“Shared governance is a structured model of empowerment gleaned from organizational development experts” (Thomas, 2017, p. 171). It is understood with shared governance that this is a “collaborative structure in which managers and clinicians’ partner to make decisions about clinical and operational practices” (Thomas, 2017, p. 171). There is a tool available for measuring professional governance, it is an 86-item Index of Professional Nursing Governance (IPNG) (Weaver, Hess, Williams, Guinta & Paliwal, 2018). The IPNG “encompasses a continuum of traditional, shared and self-governance” (Weaver et al., 2018)

From my experience in the SNF healthcare setting, it is important that leadership or managers collaborate with clinicians to develop practices and guidelines. Historically speaking with organizations in which I have worked, management are not normally clinicians; meaning that management do not always have a clinical mindset. It is important when developing plans that there is more than a business mindset, but a clinical guideline as well, when caring for patients is the primary goal.

Culture of the Organization

Culture within a healthcare organization is “perceived as an individual value shared by all, suggesting that an individual is responsible for creating and maintaining safety” (Thomas, 2017, p. 177) In making a decision in becoming a valuable asset to a healthcare organization it would be beneficial to learn about the organization’s culture. With one of the Director of Rehabilitation positions I held, I was permitted to attend a day with the current person in that position. I was able to go to daily meetings and interact within the facility and the rehabilitation staff to really get a good understanding, and realistic, of how the culture of the organization would be, if I were to come aboard.

Safety of Patient Care

In the role of Director of Rehabilitation, patient safety is a high priority for concern daily. It is typically the rehabilitation department’s responsibility to oversee the patient handling by staff members to ensure patients are being taken care of appropriately and to also prevent workplace injuries.
The safety culture of an organization is the product of individual and group values, attitudes, perceptions, competencies, and patterns of behavior that determine the commitment to, and the style and proficiency of, an organization’s health and safety management (Thomas, 2017, p. 181)

As the Director of Rehabilitation, I would want to see a structured training and assessment of staff to be sure they are following recommendations. Proper training and education limits and decrease risk of patient injury. If an organization does not place patient safety high on their priorities, then it would most likely be a deal breaker for me to accept the position as Director of Rehabilitation. With it being a low priority then it is quite likely the organization will not be open to new concepts or guidelines.

Description

As a leader it is important to be able to see both aspects of the business world. This means that as a leader you should be able to see the functional parts of your business as well as the parts of the business that are not functional. It can also be said that it is just as important for a leader to understand the culture of the organization that they are in charge of leading, so that they may be an effective leader. Our textbook states that the culture found within an organization is created from the tone of the relationships between the leaders and the followers of the organization (Neely-Barnes & Lennon-Dearing, 2017). The best way to determine where your functional dysfunctional aspects are within your organization would be to create an organizational assessment tool to help you evaluate the business.

Organizational Assessment Format

As mentioned above, it is important for a leader to know the functional and dysfunctional areas of his or her business, and to effectively do so, an organizational assessment is needed. The first area of organizational assessment that I would start with would be, shared governance. Do the people of your business feel that they are being heard and included in the implementation of new policies? This is an important part of being a leader within a business. I feel the best way to approach such a topic is to follow the same set-up demonstrated in an article concerning how nurses felt concerning their control in their workplace. The nurses were sampled and asked for their perceived decision making on 21 different topics (Gerard et Al, 2016). I choose this method because this way whoever is surveyed does not feel as though they will be penalized for giving an honest answer. If shared governance is working correctly then the workers will feel that they have a large amount of say in how their workplace is managed.

The next area in which I would look into to see if workers were satisfied would be the culture of the workplace. I believe the best way to conduct this kind of research would be to use an outside source, such as a survey website, to conduct an anonymous survey of each employee to see how comfortable they are with their boss and coworkers. Do the employees feel as though they are taken care of by the company? Do they feel they can go to their boss with an issue? Do they feel as though they can count on coworkers to help them? There are so many things within a work environment that makeup the work culture, but there are some larger items that can help change it if it’s not a positive one. I do think that the way one article conducted interviews is definitely not the way to go about it due to the fact that it is much harder for someone to be honest if they are faced with that person or worried it could get back to that person what they said (New Online, 2019). If this tool was to be seen as successful, I would expect to see fewer call out days and sick days from employees because on the whole, they are happier.

The last area in which I would do an assessment would be patient safety. I would do this from the viewpoint of the care giver, i.e. the nurses. I would do this because many times the patients may not know what our standards for patient safety are, therefore cannot give a clear definition of whether or not our safety standards are effective. There is one article that mentions we are using far too many alarms now a days and the nursing staff is becoming “fatigued” to the alarm bells (Ruskin et Al, 2015). This could pose a problem for safety regulations, and in this case I would like to cater my assessment to ask the nursing staff to fill out an anonymous report of how many safety protocols they use one patient per shift, then how we could reduce the number without compromising safety but being more efficient. My hope is that if this works effectively, that we can come up with a new way to streamline patient safety without compromising safety and without taking up too much of the nurses time.
